What we do with your information.

Treating you means holding your health information. This sets out what we collect, why we need it, who else sees it, and what you can ask us to do with it.

Your health information is the most sensitive thing you will hand us, so this page sets out plainly what we collect, why we need it, who else sees it, and what you can do about it.

Who this policy applies to

Nourish IV Therapy Pty Ltd (ABN 59 698 350 042), trading as Nourish IV Therapy ("we", "us", "our"), provides mobile IV therapy and related services across the Perth metropolitan area. This policy covers the personal information we handle when you visit this website, enquire, book, or are treated by us.

We are bound by the Privacy Act 1988 (Cth) and the Australian Privacy Principles. Private health service providers are covered by that Act regardless of their size or turnover, so the small business exemption does not apply to us.

What we collect

What we hold depends on how far you get with us. Someone who reads this site leaves almost nothing behind. Someone we treat gives us a clinical record.

  • Contact and identity details: your name, phone number, email address, and the address where you would like to be treated.
  • Health information: your medical history, current medications, allergies, relevant conditions, why you are seeking treatment, what was administered, and how you responded. Health information is sensitive information under the Privacy Act and attracts the highest level of protection in it.
  • Booking and payment records: the service booked, the date and time, the deposit, and invoices. Card numbers are entered into our booking and payment provider and are never held by us.
  • Website and technical information: this site runs no advertising, no analytics and no tracking, and sets no cookies of its own. Our booking system opens in a pop-up on the site and may set its own cookies, which are governed by that provider's policy. Our hosting provider keeps ordinary server logs, which can include your IP address.

How we collect it

  • Directly from you: on the free phone consultation, at your appointment, through the booking system, or by email or social media.
  • Through our booking provider, when you book a time and pay the deposit.
  • From a third party such as your general practitioner, a prescribing medical practitioner or a pathology provider, where you have asked us to or agreed that we can.

We collect health information only with your consent, and only where it is reasonably necessary to treat you safely. You can decline to give it. If you do, we may not be able to treat you, because the assessment is what makes the treatment safe.

Why we collect it, and what we use it for

  • To assess whether a treatment is appropriate and safe for you.
  • To formulate, administer and record your treatment, and to arrange the medical oversight and prescriptions it requires.
  • To confirm appointments, contact you about a booking, and follow up afterwards.
  • To take the deposit, issue invoices and collect payment.
  • To keep the clinical, tax and business records we are required by law to keep.
  • To answer your questions, and to handle feedback or a complaint.

We will only send you marketing material if you have agreed to receive it, and every message will carry a way to opt out. We do not sell personal information, we do not trade it, and we do not disclose it for anyone else's marketing.

Who else sees it

We disclose personal information only where it is necessary, and only to:

  • The prescribing or supervising medical practitioner involved in your care.
  • Pathology and laboratory providers, where you have had blood testing.
  • Your general practitioner or another treating practitioner, where you have asked us to.
  • Service providers who help us run the business: our booking and payment platform, email and website hosting, and our accountant. They may use the information only to provide that service to us.
  • Anyone else you have consented to, and anyone we are required or authorised by law to tell, including in an emergency where your life or health is at risk.

Some of those service providers store data on servers outside Australia. Where that happens, we take reasonable steps to ensure the information is handled in a way consistent with the Australian Privacy Principles.

Storage, security and how long we keep it

We take reasonable steps to protect personal information from misuse, interference and loss, and from unauthorised access, modification or disclosure. Records are held electronically in access controlled systems, and any paper record is stored securely.

Health records are kept for at least seven years from the date of the last entry. Where the client was under 18 at the time of treatment, the record is kept until they turn 25. That is the retention period expected of Australian health service providers. Once information is no longer needed and we are not required to keep it, we destroy or de-identify it.

No system is completely secure. If a data breach occurs that is likely to result in serious harm, we will notify you and the Office of the Australian Information Commissioner, as the Notifiable Data Breaches scheme requires.

Seeing and correcting your information

You can ask for a copy of the personal information we hold about you, and you can ask us to correct anything inaccurate, out of date or incomplete. Email info@nourishivtherapy.com.au and tell us what you need.

We will verify your identity first, and we will respond within 30 days. Access is normally free, although we may charge a reasonable fee for the cost of retrieving and copying a large or complex request. In the limited cases where the Privacy Act permits us to refuse access, we will tell you why in writing.

Complaints

If you think we have mishandled your personal information, tell us first at info@nourishivtherapy.com.au. We will acknowledge the complaint, look into it, and respond within 30 days.

If you are not satisfied with how we handle it, you can take the complaint to the Office of the Australian Information Commissioner at oaic.gov.au, or on 1300 363 992.

Changes to this policy

We may update this policy as the service or our obligations change. The current version is always the one on this page, and the date it was last updated is shown at the top of it.
